Also known as
reduction mammaplasty, this is an operation intended to reduce
the size of a woman's breasts and improve their shape and
position.
Frequently, the areola (dark skin around the nipple) is also
made smaller. Functional symptoms (medical disorders) caused by
excessive breast weight may be relieved or improved by this

The first consultation

Where are the incisions?
This is probably the most important topic that gets discussed in
relation to Breast lift and Breast Reduction. Plastic surgery
does leave scars and it is important that you see plenty of
postoperative photos to understand the spectrum of scarring that
can occur with breast surgery.
Types of incisions and techniques:
There are a variety of techniques for breast reduction
(reduction mammaplasty) procedure. The techniques reflect a
combination of differing skin incisions (scar pattern) and the
methods used in shaping the breast substance.
Liposculpture - This technique is not commonly used.
However, in patients who have large breasts that are mainly
fatty in nature and do not exhibit droopiness (ptosis),
liposuction can be quite successful with almost no scarring.
The European technique involving an incision around the
areola (BENNELLI'S-GOES). This method is usually reserved for
breast lifting and a very small breast reduction.
The French-Canadian breast reduction technique involves
the extra incision between the areola and breast crease as well
as the one around the nipple (lollypop scar, LE JOUR and
HALL-FINDLAY).
This is a method of reducing small-moderate size breasts in
generally younger patient (20-50 range). It produces remarkably
perky breasts with excellent long term results.
The
final technique involves the above incisions, plus the extra
long incision within the crease under the breast (WISE pattern-
most traditional). This is an "anchor-shape" or inverted "T"
incision. Our surgeons still utilize this method in very large
to gigantic breast reduction cases especially in the older age
group (50-70).
Occasionally, especially with Breast lifting only, these
incisions may be modified and more limited. Some operations
may require only the incision around the areola.
Anesthesia
Local anaesthesia and complementary sedation, or general
anesthesia, depending on the extent of the procedure and on what
you and your surgeon prefer.
Duration of the operation
Between 1H30 and 2H30.
How long is the patient hospitalized?
For a breast reduction, one day of hospitalization is usually
needed if there are no complications.
Usually suction drains (plastic tubes) are left in place after
surgery with breast reduction, and possibly with a Mastopexy.
Recovery
Discomfort, swelling, and discoloration of the breasts are to be
expected for several weeks. Usually, our patients return to
almost normal activity within two weeks. The scars at the
incision lines typically become reddish, raised, and firm a few
weeks after surgery, but after many months become pale and soft.
After 8-12 months, the scars are relatively inconspicuous. The
nipples and some areas of the skin may be numb or sensitive
after surgery. Sensation frequently returns within a few weeks
or months but may be diminished or overly sensitive.
What are my
limitations in activity post-operatively?
You should plan to avoid activities which require much raising
of the arms above the level of the head for 10 days after
surgery. With great care, you can drive about 7 days after
surgery. Patients can usually return to work in a few days
unless their occupation requires particularly strenuous
movements and lifting. In such cases, 2-3 weeks should be
allowed.
Will the breasts
start to sag again?
Gravity
continues to have its effect, and there is a tendency for the
skin of the breast to stretch over a long period of time. Women
vary a great deal in this respect. In general, the smaller the
breasts, the less tendency for sagging to recur. If the breasts
sag further, excision of the skin on an outpatient basis can be
used to correct the problem. If we try to lift heavy breasts
without making them smaller at the same time, sagging will
return soon. One key to a satisfying result is realistic
expectations - a wide-based large breast will not look like a
smaller, but firm, narrow-based breast.

The term comes from the Greek words gyne
meaning "woman" and mastos meaning "breast." In practical terms,
this means abnormally large breasts on men.
The condition is relatively common in adolescent boys, and 90%
of the time symptoms disappear in a matter of months, or, as
adolescence wanes, a few years later. But the remaining 10% are
burdened with a social handicap that causes a deep and complex
shame, and puts one's relationship with one's body at risk.
There are several
potential causes:
• puberty
• steroid abuse (bitch tits)
• obesity
• marijuana use (this is in question)
• tumors
• genetic disorders
• chronic liver disease
• side effects of many medications
• castration
• Klinefelter Syndrome
• Gilbert's Syndrome
• aging
The Remedy
In cases of obesity, weight loss can alter the gynecomastic
condition, but for many it will not eliminate it. For all other
causes, surgery is the only known physical remedy. Once the
physical encumbrance is lifted, psychological scars still need
to be addressed. One must come to terms with one's body, accept
it, and heal the wounds from the past.
Psychological Issues
Gynecomastia can be emotionally devastating. Feelings
of shame, embarrassment and humiliation are common. One does not
feel masculine in a society where masculinity is exalted. Self-hate
threads itself through all aspects of the individual's life,
creating an insidious web of powerlessness. A man or boy with
gynecomastia struggles with anxiety over such simple acts as
taking off his shirt at the beach.
For many men, the best solution is surgery. That accomplishes
step one of the healing. Step two is psychological redress. From
childhood taunting to a lifetime of hating his chest, the hurt
feelings will not go away with the fact of breast reduction
alone.
Men who have developed gynecomastia later in life from steroid
abuse or some other cause may have little to no psychological
distress. However, for some in this situation, it can leave them
feeling out of control of their body or emasculated in some
matter. Hopefully, corrective surgery will resolve these
feelings, for some it will not and therapy will needed to
relieve the distress.
It is important to recognize the scars on the inside. This is
difficult work because it means coming to terms with one's body
and past. Acknowledging the pain, moving into a new relationship
with one's body and changing how he thinks the world sees him is
the key to healing and freedom.
Men often have a very difficult time talking about their breasts
to anyone, but it is the first step toward relief. Realizing
that they are not alone is a powerful antidote for the shame and
a beginning toward healing.

Procedure
The
most common technique for GRS is where the surgeon makes a
peri-areolar incision to remove glandular tissue and then
performs liposuction of the area for definition. Some surgeons
only use liposuction, but in most cases I think this is a
mistake unless it is pseudo-gynecomastia (all fat content).
Twenty percent of my practice is re-do surgery from unhappy
patients. This is most often due to the technique of liposuction
only. This leaves the glandular tissue behind.

Between 1 hour and 2 hours
How long is the patient hospitalized?
This procedure can be done as an outpatient procedure requiring
no hospitalization.
Recovery
The initial recovery period is the 7 to 10 days right after your
procedure. If drains are used, they will typically remain in for
about three days after which they will be removed at your first
post-op appointment.
Total recovery time can vary from patient to patient. Most
patients can expect to be fully healed by 3-6 months. At this
point most or all of the bruising and swelling should have
dissipated and the scars should have begun to shrink and fade.
Sometimes, it may take as long as a year to completely heal to
the point where the scars are no longer visible.
For the first 4 to 6 weeks after surgery, it is necessary to
avoid all strenuous activities, especially those involving the
upper body such as lifting weights or intense workouts at the
gym. In addition We advise our patients to wear a compression
garment for 6 weeks post-operatively.
You as a patient have invested a lot of time and money into this
procedure, and as such you owe it to yourself to also invest the
time and patience needed for healing in order to achieve optimum
results.
